VHW Visit Verification Form

HMK Visit Verification Form is a utility app for VHW homemakers to electronically verify their visits.  Using this app, homemakers can electronically provide visit dates, visit start and visit end times, what tasked were completed during the visit, and have client signature to confirm the visit and tasks performed. The app allows only prescribed user to login and complete the form, which results in the emailed report.
